Sometimes confidential patient data is needed for research and audit fror future health care provision and for teaching or training purposes. We only ever provide this information where there is a direct medical need for it and we always remove any details which could identify you as an individual unless we have first asked for your written permission to give them.

18-24yrs -       $42.00
25yrs & Over - $42.00

- patients who are not enrolled
- patients who are enrolled but not yet funded
- nursing services
